GLP-1 Medications and Fertility: What Patients Need to Know

As a reproductive endocrinologist, I frequently work with individuals and couples navigating complex fertility challenges. Many of these challenges—particularly those related to polycystic ovary syndrome (PCOS), irregular ovulation, or unexplained infertility—are influenced by underlying metabolic health. In recent years, a class of medications called GLP-1 receptor agonists has gained attention for their impact on weight loss and insulin regulation, and naturally, many patients have begun asking: Can these medications help improve fertility?

This is an important and evolving topic. While GLP-1 medications were originally developed for type 2 diabetes, they are now widely used to manage obesity and metabolic syndrome. For some patients, these medications may offer benefits that extend beyond blood sugar control, particularly in optimizing conditions that support reproductive health.

Understanding GLP-1: What Are These Medications?

GLP-1 stands for glucagon-like peptide-1, a hormone naturally released by the gut in response to food intake. It plays a key role in regulating blood sugar, slowing gastric emptying, increasing insulin secretion, and promoting feelings of fullness. GLP-1 receptor agonists mimic this hormone, helping patients manage weight and metabolic conditions more effectively.

The Connection Between GLP-1 and Female Fertility

One of the most promising areas of research around GLP-1 use in fertility is among patients with polycystic ovary syndrome (PCOS). PCOS often involves insulin resistance, anovulation, and elevated androgen levels (such as testosterone), all of which interfere with the normal hormonal signals required for ovulation and conception.

Weight loss plays a crucial role in improving fertility for women with PCOS. Research shows that losing just 5–10% of body weight can restore ovulation and enhance fertility outcomes. GLP-1 medications can be an effective aid in achieving this weight loss. Several studies have demonstrated that women with PCOS who use GLP-1 receptor agonists experience greater improvements in menstrual regularity and ovulation rates compared to those relying on lifestyle changes alone. In addition to weight loss, GLP-1 therapy may reduce circulating insulin and testosterone levels, which can further support hormonal balance and ovulatory function.

That said, it is important to emphasize that GLP-1 medications are not approved by the FDA for use as fertility treatments. While emerging research is promising, we do not yet have sufficient long-term data to fully understand how these medications impact egg quality, embryo development, or pregnancy outcomes. Therefore, I consider them an adjunct therapy to improve the underlying metabolic environment before fertility treatment, rather than a primary fertility medication.

Safety Considerations: Planning Around Pregnancy

One of the most important points I discuss with patients is that GLP-1 medications are not considered safe during pregnancy. Animal studies have shown potential risks to fetal development, and human data remain limited. As a precaution, current guidelines recommend stopping GLP-1 medications at least one to two months before attempting conception, whether trying naturally or through fertility treatment.

What About GLP-1 and Male Fertility?

While most of the research on GLP-1 and fertility has focused on women, early studies suggest there may be benefits for men as well. Some data indicate that GLP-1 receptor agonists can help improve testosterone levels, particularly in men with obesity or type 2 diabetes. This could, in turn, support better sperm quality, libido, and overall reproductive function. However, much more research is needed in this area before we can draw firm conclusions or incorporate GLP-1 therapy routinely into male fertility care.

Final Thoughts: A Promising Tool, But Not a One-Size-Fits-All Solution

GLP-1 medications are a powerful option for addressing metabolic challenges that can affect fertility, especially in patients with PCOS or obesity.
